Service Planning Coordinator

Simcoe Community Services – Barrie – Ontario

We are presently accepting resumes for the position of permanent full-time Service Planning Coordinator. This position supports families of children and youth with multiple and/or complex special needs by acting as the identifiable point of contact; is responsible for developing a coordinated service plan that recognizes all service needs and builds on the family and child/youth's strengths.  Assist families to connect with the right services in their service delivery area and monitor how the family and child/youth is progressing through the service plan as they grow. To act as the single point of contact for all service providers on the child/youth and family team and be responsible for communication and information sharing with all relevant service providers. In this role, you will be working in collaboration with Children's Treatment Network of Simcoe York to ensure service integration for children, youth and their families experiencing multiple and/or complex special needs. This position works up to 37.5 hours a week and has a rate of pay of $29.25 per hour. 
OVERVIEW OF DUTIES:

Use child/youth and family centred service delivery best practices to guide service delivery
Assist families to identify needs and strengths through the use of an evidenced based tool (CANS)
Assist family/caregiver and child/youth to identify their vision and goals
Work with child or youth, their family and a multidisciplinary team to develop and implement a Coordinated Service Plan;
Ensure the Coordinated Service Plan is integrated and identifies specific actions to achieve goals;
Record all contact and activities on the Child/Youth's Electronic Record
Facilitate case conferences and work closely with multidisciplinary team to ensure the family and child/youth are experiencing integrated service delivery;
Be actively involved in a process of continuous quality improvement with respect to serving families;
Understand and implement Personal Outcome Measures;
Participate in Service Planning Coordinators Community of Practice

MINIMUM REQUIREMENTS OF THE JOB INCLUDE:

University degree in a relevant course of study (e.g. social work, psychology, nursing, child studies, family studies, or rehabilitation) or an equivalent combination of education and experience as deemed acceptable by the Agency. 
A minimum of 3-4 years related experience with families and their children/youth;
Demonstrated excellent verbal and written communication and interpersonal skills;
Proficient computer skills and database administration;
Knowledge of developmental norms (0-17); 
Skilled in various assessment tools to determine family and child's strengths/needs; 
Knowledge of community based supports; 
Proven ability to work with a multidisciplinary team and independently;
Must be prepared to work flexible hours in order to meet the needs of children, youth and families;
Must possess a valid class "G" driver's license and own reliable transportation is required;
Must be willing to travel through Simcoe County

Housing Access Worker

We are recruiting for a full-time contract Housing Access Worker to assist individuals and families who are homeless or at risk of homelessness to obtain affordable accommodation and relevant support services; to identify individuals with chronic housing needs and requiring longer term assistance; to operate the housing registry and recruit and support landlords for the registry; providing landlord/tenant mediation and to work with the Housing Tribunal. This position works up to 37.5 hours per week in Orillia and surrounding areas, and has an hourly rate of pay of $25.13. 
Minimum requirements of this position include:

preference will be given to applicants with a degree/diploma in the field of social services and who have recent relevant experience working with the target population; 
up to one year's full-time experience in promoting housing for marginalized persons, experience within the Addictions, Mental Health or a closely related field is an asset
demonstrated knowledge of human/legal rights and relevant legislation; 
knowledge of screening and assessment tools related to the field;
demonstrated excellent communication and assessment skills; networking skills; 
knowledge of community resources;
the ability to work independently and as part of a team are required; 
a valid driver's license and own reliable transportation to be used in the course of employment is required; 
must be prepared to do some evening and weekend work;

Duties and responsibilities:  (including, but not limited to) 

maintaining a caseload of individuals and families who require assistance in obtaining and maintaining adequate housing;
advocating to landlords on behalf of clients requiring housing;
developing and maintaining a housing registry of persons willing to rent rooms, apartments, houses to persons with exceptional housing needs;
providing support to both landlords and tenants, in order to maintain housing;
facilitating information sharing workshops and sessions;
providing training in budgeting, household management, social skills, and other skills necessary to maintain adequate housing;
being familiar with Landlord-Tenant Act and other related regulations and assisting clients accordingly;
assessing housing needs and assisting clients in securing housing to meet those needs;
