Transition Services

Transition supports are available to youth and young adults with an intellectual disability who are attending or who have recently left high school.  The Transition Coordinator acts as a support and provides resources to the person, their family, schools and other community agencies.   Skills and abilities are assessed, interests are explored through the Essential Lifestyle Planning Model to enhance individual goals of decision making, problem solving, daily living skills (shopping, cooking budgeting, safety awareness, etc) all leading to successful community participation.   The initiative will focus on informed choices to develop and maintain active and meaningful community involvement including leisure activities, recreation options, further education, volunteer opportunities, work experience and independent living based on the person’s own preferences. Transition services are offered in Barrie, Orillia and surrounding areas.  Transition supports are governed by the Developmental Services Act and funded through the Ministry of Community and Social Services.  Referrals are accepted from individuals, families, schools and other community agencies.  For more information contact 727-1234 ext. 240 or 222 in Barrie and 327-5391 ext. 252 in Orillia.
