Transition Supports Services Coordinator

Simcoe Community Services – Barrie – Ontario

Full Time - $27.15 - Hourly

We are recruiting for a permanent full-time Transition Supports Services Coordinator, to provide advocacy and support in collaboration with youth, their families, schools and community to ensure young people with intellectual disabilities have a planned and coordinated transition from school to work, further education and participation in life activities.  This position works 37.5 hours per week with a rate of pay of $27.15 per hour. 
MINIMUM REQUIREMENTS OF THE JOB INCLUDE:

degree or diploma in developmental services or a closely related field; or a combination of education and experience which provides the required knowledge, skills and abilities;
two years recent full time work experience working with teens and young adults with intellectual disabilities; familiar with Person Centered Planning, Personal Outcome Measures and Outcome based practices;
good knowledge of various community resources and agencies;
excellent written and verbal communication skills, above average interpersonal and collaborative abilities combined with good computer skills and experience in Microsoft office applications and database administration;
valid class "G" driver's licence and own reliable transportation is required to be used in the course of employment;
current valid certificates and a demonstrated proficiency in First Aid, C.P.R., and N.V.C.I. is required;
will be required to work flexible hours which will include working some evenings, weekends and holidays;

OVERVIEW OF DUTIES: (Not a complete list of duties)

assisting individuals to make informed choices that lead to successful transition from school;
working directly with individuals with intellectual disabilities and their families in developing a transition plan from school to adult living;
understanding what is important to the individual's life and coordinating supports to work towards  achieving their future goals; 
working directly with the individual and family in developing a time limited service plan;
working with the individual and family regarding the potential acquisition of a support worker;
liaising and developing strong and effective partnerships with a wide range of local community resources including the local secondary schools, employment agencies, community services, volunteer organizations and others as necessary;
producing outcome oriented quarterly statistics and data as required to demonstrate the impact of the services on local individuals and families;
