Transition Services

Available to youth and young adults with an intellectual disability who are attending or who have recently left high school. The Transition Coordinator acts as a support and provides resources to the person, their family, schools and other community agencies. Skills and abilities are assessed, interests are explored through the Essential Lifestyle Planning Model to enhance individual goals of decision making, problem solving, daily living skills (shopping, cooking budgeting, safety awareness, etc) all leading to successful community participation.

The initiative will focus on informed choices to develop and maintain active and meaningful community involvement including leisure activities, recreation options, further education, volunteer opportunities, work experience and independent living based on the person’s own preferences.

Transition services are offered in Barrie, Orillia and surrounding areas.

Transition supports are governed by the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act, 2008, and funded through the Ministry of Community and Social Services.
